The services offered include an Initial Health Assessment, Health and Wellness Coaching, Group Classes, and Interactive Discussions to obtain the following:

Once you schedule an appointment, kindly take the next step and complete the
Initial Assessment Questionnaire and also take time to fill out the Food Journal.
In addition, once a session starts whether group or individual, you are required to sign the DIsclaimer Form.
